I am voluntarily choosing to work with the practitioner and acknowledge that the information and guidance provided are for educational purposes only. I understand that it is not a substitute for medical advice and that I take full responsibility for my health decisions. I agree to hold the practitioner harmless for any outcomes resulting from my actions.

By agreeing to this consent form, I release the practitioner from any liability associated with the services provided and any actions I take based on the information and guidance received.
